Specialized Healthcare Brokerage

Selling a healthcare agency in Virginia

Riverbluff Advisors is a healthcare business broker that sells independent, owner-operated home care, home health, and hospice agencies in Virginia. We work with owners across the state, from Northern Virginia and Richmond to Hampton Roads, Roanoke, and the Shenandoah Valley.

Virginia agency types we sell

  • Home care: non-medical personal care, often paid through Virginia Medicaid's Cardinal Care managed care plans. Buyers look at weekly hours, caregiver retention, and which plan contracts move with the sale.
  • Home health: Medicare-certified agencies. Buyers look at episode volume, referral sources, and whether the Medicare enrollment can transfer.
  • Hospice: buyers price census, cap position, and survey history.

How a Virginia agency sale works

Virginia licenses home care organizations. The Virginia Department of Health's Office of Licensure and Certification (OLC) licenses home care organizations. A change in operator must be reported to OLC in writing 30 working days in advance, and OLC decides whether the buyer needs to file a new application.

OLC may inspect. OLC reviews the planned change and may inspect the agency before reissuing the license. We make sure the agency's records are in order before it goes to market.

Medicare-certified agencies have a second clock. For home health and hospice, the federal change of ownership rules apply on top of the state process, including the CMS 36-month rule.

Medicaid contracts set the timeline for home care. Agencies paid through Cardinal Care plans usually need the buyer credentialed or the plan's consent before closing. We review every contract up front so the closing date holds.

Virginia seller questions

How long does a Virginia agency sale take?

Most take three to eight months. Virginia requires 30 working days' notice before a change in operator, and Medicare and Medicaid approvals usually set the final timeline.

Can I sell a home care agency with Medicaid clients?

Yes. Agencies with steady Medicaid hours are in demand. The buyer usually has to be credentialed with each plan, so we confirm what each contract requires before going to market.

Do I need a Virginia attorney?

Yes. We coordinate with your attorney on the purchase agreement and filings, but we do not give legal advice.

What does it cost?

Nothing until closing. Our fee is a success fee paid when the sale closes.
