Change of ownership and regulatory considerations

These items set the closing date on a medicare-certified hospice transaction. We check each one before your agency is marketed so a buyer never finds the problem first.

Change of ownership

A hospice CHOW is filed on CMS 855A within 30 days of the effective date. The buyer accepts the provider agreement as it stands, including successor liability for prior overpayments, which is why cap exposure is quantified before the LOI.

Medicare PTAN transfer

The PTAN follows the provider agreement when the buyer accepts assignment. Rejecting assignment means a new PTAN and initial certification, so the buyer normally accepts. Expect 60 to 120 days for the MAC to complete processing.

Licensing timeline

State hospice licensure change of ownership typically runs 60 to 90 days and is the item most likely to set the closing date. Accreditor notification to CHAP or ACHC triggers a post-transaction visit.

  • CHAP or ACHC accreditation carries over but the accreditor must be notified and will schedule a visit.
