Selling a healthcare agency in Ohio

Riverbluff Advisors is a healthcare business broker that sells independent, owner-operated home care, home health, and hospice agencies in Ohio. We work with owners across the state, from Columbus, Cleveland, and Cincinnati to Dayton, Toledo, and Akron.

Ohio agency types we sell

  • Home care: non-medical personal care, homemaking, and respite. Many Ohio agencies are paid through Medicaid waiver programs such as PASSPORT. Buyers look at weekly hours, caregiver retention, and which payer contracts move with the sale.
  • Home health: skilled home health agencies, many of them Medicare-certified. Ohio has one of the largest home health markets in the country, especially around Columbus.
  • Hospice: buyers price census, cap position, and survey history.

How an Ohio agency sale works

Ohio licenses home care and home health agencies. Since 2022, the Ohio Department of Health (ODH) has licensed both skilled home health and non-medical home care agencies. The buyer needs its own ODH license to operate the agency, so we plan that filing into the timeline from the start.

Medicaid waiver contracts move separately. Agencies paid through PASSPORT and other Ohio Medicaid waivers need the buyer certified or enrolled with the program before it can bill. We review every payer contract up front so the closing date holds.

Medicare-certified agencies have a second clock. For home health and hospice, the federal change of ownership rules apply on top of the state process, including the CMS 36-month rule.

Ohio's market is crowded, which makes positioning matter. With hundreds of home health agencies and thousands of home care agencies, buyers have choices. A clean record, steady hours, and organized financials are what make an Ohio agency stand out.

Ohio seller questions

How long does an Ohio agency sale take?

Most take three to eight months. The buyer's ODH license, Medicare filings, and Medicaid waiver enrollment usually set the timeline.

Can I sell a home care agency that serves PASSPORT clients?

Yes. Agencies with steady waiver hours are in demand. The buyer has to be certified or enrolled with the program, so we confirm what each payer requires before going to market.

Does Ohio require a certificate of need to buy a home health or hospice agency?

No. Ohio does not require a certificate of need for home health or hospice agencies.

Do I need a Ohio attorney?

Yes. We coordinate with your attorney on the purchase agreement and filings, but we do not give legal advice.

What does it cost?

Nothing until closing. Our fee is a success fee paid when the sale closes.
