Selling a healthcare agency in Louisiana

Riverbluff Advisors is a healthcare business broker that sells independent, owner-operated home care, home health, and hospice agencies in Louisiana. We work with owners across the state, from New Orleans and Baton Rouge to Lafayette, Shreveport, and Lake Charles, and our Texas buyer network extends naturally into Louisiana.

Louisiana agency types we sell

  • Home care: personal care attendant and other home and community-based services, licensed by the Louisiana Department of Health and often paid through Long Term Personal Care Services (LT-PCS) or the Community Choices Waiver. Buyers look at weekly hours, caregiver retention, and which payer contracts move with the sale.
  • Home health: Medicare-certified agencies. Buyers look at episode volume, referral sources, and whether the Medicare enrollment can transfer.
  • Hospice: buyers price census, cap position, and survey history.

How a Louisiana agency sale works

Your license does not transfer to the buyer. Louisiana home and community-based services licenses cannot be transferred, assigned, or sold. The buyer applies to the Louisiana Department of Health (LDH) for its own license through a change of ownership.

The buyer has to show it can operate. LDH requires the buyer to show a line of credit of at least $50,000, general and professional liability insurance of at least $300,000, and workers' compensation coverage. We screen buyers for this before they see your agency.

The agency must be in good standing. LDH will not process a change of ownership if the license is provisional, under revocation, under a settlement agreement, or excluded from Medicaid. We check this before your agency goes to market.

Medicare-certified agencies have a second clock. For home health and hospice, the federal change of ownership rules apply on top of the state process, including the CMS 36-month rule.

Medicaid programs set the timeline for home care. Agencies paid through LT-PCS or the Community Choices Waiver need the buyer enrolled before it can bill. We review every program and contract up front so the closing date holds.

Louisiana seller questions

How long does a Louisiana agency sale take?

Most take three to eight months. The buyer's LDH license, Medicare filings, and Medicaid enrollment usually set the timeline.

What does a buyer need to take over my Louisiana license?

Its own LDH license application, plus proof of a $50,000 line of credit, liability insurance, and workers' compensation coverage. We screen buyers for this up front.

Do I need a Louisiana attorney?

Yes. We coordinate with your attorney on the purchase agreement and filings, but we do not give legal advice.

What does it cost?

Nothing until closing. Our fee is a success fee paid when the sale closes.
